Enrolment Procedure

Enrolment Procedure for New Students (SY 2012-2013)

Step 1

Submit complete requirements to Admission/Registrar’s Office for Evaluation:

  • Form 138 (Report Card)
  • Birth Certificate Copy
  • Certificate of Good Moral Character
  • Passport and Residence Permit Copies (Student and Parents)
  • Two (2) passport size pictures

Step 2

The Registrar will issue an Entrance Test Form to be presented to the Cashier for payment of QR.

Step 3

Present the Entrance Test Form and the receipt to the Guidance Office.
Guidance Office will inform the enrollee when to secure enrolment permit.

Step 4

Secure enrolment permit from the Guidance Office.
Present the enrolment permit to the Admission/Registrar’s Office and secure registration form.

Step 5

Accomplish Registration Form, and submit to the Admission/Registrar’s Office for verification of enrollee’s data. Test.

Step 6

Proceed to the Cashier for tuition and other school fees assessment.
Upon payment of required fees, student is deemed enrolled.

Step 7

Present receipt of payment for books to Textbook Release Section.
